Fall Prevention

San Joaquin County’s Fall Prevention Program is a no cost program offering information and education about injury prevention, referrals and provision of fall and injury prevention resources, in-home environmental assessments, and instruction on behavioral, physical, and environmental aspects of injury prevention. Examples of equipment include grab bars, toilet safety frames, non-skid shower mats, shower chairs, threshold ramps, and much more. Services are provided to individuals living at home and who have experienced falls or are at risk of falling. Eligibility is NOT based on income.

Eligibility Requirements

  • Age 60 years old
  • Has fallen, is at risk for falling or is at risk for institutionalization
  • Participants must be able to continue to live independently
  • Reside in San Joaquin County

Health Insurance Counseling Advocacy Program (HICAP)

HICAP provides free and objective information and counseling about Medicare. Volunteer counselors can help you understand your specific rights and health care options.

HICAP counselors can:
- Explain Medicare benefits and plan options
- Assist with cost-saving options
- Help with understanding long-term care services and support
- Advocate for denial of medical coverage and problems with payments to providers
- Assist with Medicare appeals and referrals for legal assistance
- Assist with Medicare fraud and abuse referrals
- Provide Community education, including Roadmap to Medicare classes

In-Home Supportive Services

The In-Home Supportive Services (IHSS) program will help pay for services provided to you so that you can remain safely in your own home. To be eligible, you must be over 65 years of age, or disabled, or blind, and meet income and resources limits for the program. Disabled children are also potentially eligible for IHSS. IHSS is considered an alternative to out-of-home care, such as nursing homes or board and care facilities.

Case Management Program

Our Case Management Program is designed to support older adults, sixty (60) years of age or older, by providing services that promote independence and well-being. The program offers tailored case management, helping to coordinate care and link individuals to necessary community resources. This approach ensures that residents receive the support they need to live safely and comfortably within their community.

Older Adult HomeMeds and Medicare Part D Outreach Clinic

Provided by: University of the Pacific - Thomas J. Long School of Pharmacy
Eligibility: individuals who are 60+ years

Services provided:

  • Medication Therapy Management: a comprehensive evaluation of the patient's medications to ensure they are safe and effective to take together.
  • Medicare Part D evaluation: provide an objective, unbiased evaluation of all Part D prescription drug plan offerings to identify the one that will help minimize out-of-pocket prescription drug costs.

Older Adult Legal Assistance

Provided by: Senior Advocacy Network
Eligibility: individuals who are 60+ years

Legal assistance is prioritized for older adults who cannot afford traditional legal services, such as:

  • Foreclosure prevention/relief from wrongful foreclosure
  • Consumer debt
  • Tenant Rights:Notices to quit, Reasonable Accommodation, Eviction Defense, Rental Increases, Habitability
  • Advance Care Planning:wills, Powers of Attorney, Advance Healthcare Directives
  • Elder Abuse Prevention:Elder Abuse Restraining Orders, Post-scam counseling

Older Adult Homemaker, Personal Care, and Respite Services

Provided by: Catholic Charities and Jene Wah
Eligibility: individuals who are 60+ years

Services provided:

  • Homemaker: Housekeeping (i.e. light housekeeping, laundry, clean bathroom etc.)
  • Personal Care: Provides assistance with bathing and dressing.
  • Respite Care: Provides relief for the primary caregiver who lives in the home with the older adult (60+ years) (Catholic Charities only).

A Matter of Balance is an evidence-based, field-tested fall prevention program shown to be highly effective in reducing the fear of falling and increasing activity levels among older adults.

Participants learn:

  • View falls as controllable
  • Set goals for increasing activity levels
  • Make changes to reduce fall risk at home
  • Exercise to increase strength and balance

Join the Older Adult Lunch Program for nutritious meals in a community setting, supporting health and social connections for older adults aged 60 and older.
Brown Bag Program
The Brown Bag Program is designed to deliver bags of nutritious supplemental groceries to low-income older adult citizens aged 60 and over.
