About Fall Prevention

San Joaquin County’s Fall Prevention Program is a no cost program offering information and education about injury prevention, referrals and provision of fall and injury prevention resources, in-home environmental assessments, and instruction on behavioral, physical, and environmental aspects of injury prevention. Examples of equipment include grab bars, toilet safety frames, non-skid shower mats, shower chairs, threshold ramps, and much more. Services are provided to individuals living at home and who have experienced falls or are at risk of falling. Eligibility is NOT based on income.

ELIGIBILITY

Eligibility Requirements:

  • Age 60 years old or adult with a disability
  • Has fallen, is at risk for falling or is at risk for institutionalization
  • Participants must be able to continue to live independently
  • Reside in San Joaquin County

HOW TO APPLY FOR SERVICES
